Marac to Marac transfer form

A one page downloadable template of the Marac to Marac transfer form

This transfer form should be used by practitioners during a Marac to Marac referral. A referral occurs when a victim of domestic abuse moves between geographic areas, whether temporarily or permanently, and is therefore referred from one Marac to another Marac.

The transfer form assists in the referral process. It covers details of:

  • the referring area
  • the victim’s information
  • the current Idva service information
  • any other relevant information.

Marac referrals should only be sent using secure email or other secure methods.
