Referral into Idaa service form

Use this form to make a referral, offering those assessed as being at high risk of serious harm the opportunity to access specialist support

Referring to an Idaa service offers those assessed as being at high risk of serious harm the opportunity to access specialist independent support. 

Idaas:

  • coordinate a multi-agency response to a victim of domestic abuse assessed to be at high risk of serious harm or homicide
  • provide independent representation and advocacy for victims of domestic abuse through the Marac process
  • are the victim’s voice at the Marac meeting.

Use this form to make a referral to an Idaa service. When completed the form should be sent by secure means to the correct Idaa service for your area. Please note, forms should not be emailed to SafeLives.
